TALENT RELEASE FORM – Our Lady of Grace Church, Encino, CA - Viral Video
I hereby certify that I am of eighteen years old, (or accompanied by parent or legal guardian, or I am the parent or guardian of the child or children in this photo or video, and give full permission to enter in this agreement and the terms stated) and have every right to contract in my own name, or with permission from my legal guardian or parent, and I hereby give an irrevocable grant to, Our Lady of Grace Church, Encino, CA, of a free and unlimited right and permission to use, sell, distribute, publish and republish, televise or broadcast, and to copyright, for any and all educational, news, trade, publicity, and commercial or other purposes, slides, photographs, and pictures including my likeness, movies of my voice and my written and/or spoken statements, and any such additions, alterations, editing or changes therein to sell, use, distribute, broadcast, publish and republish, and/or rebroadcast the same, either separately or together with my name or a fictitious name, my address, title, and affiliation.
I hereby consent to the recording, and use and reuse by, Our Lady of Grace Church, Encino, CA, and/or any other broadcasters, and any of their respective licensees, assigns, parents, subsidiaries, divisions, business units, or affiliated entities and each of their respective employees, agents, officers and directors (collectively “Releasees”) of my performance (or photo) in the Program, including my voice and actions, my likeness, name, appearance and biographical material (collectively “Likeness”) in any and all media now known or hereafter devised, worldwide, including the world wide web and the internet, in perpetuity, in any and all versions now known or hereafter devised (including digitized versions) in, or in connection with, the Program.
In addition, I further grant to, Our Lady of Grace Church, Encino, CA, the right to copyright such pictures and images in its own name or to publish, to market and to assign without further consideration, compensation or report to me.
I hereby waive any rights or interests that I might have in the pictures and images, including any rights to inspect and/or approve the finished photographs/video and images or the use of which they may be applied so long as their use shall be lawful.
I, on behalf of myself, my family, heirs, and personal representatives, expressly release, discharge and agree to hold harmless, Our Lady of Grace Church, Encino, CA, representatives, agents, employees and assigns from and against any and all claims, demands and causes of action which I might have arising from any actual or alleged violation or infringement of any trademark, trade name, contract, agreement, copyright (common law or statutory), patent, invasion of privacy, defamation, or any other cause of action arising out of the production, distribution and exhibition of the photographs/video and images.
I agree that this Release shall be construed pursuant to the laws of the State of California. Venue for any legal proceeding concerning this Release shall be in Los Angeles, California.
